Imagine Me Leadership Charter School welcomes your son for enrollment for the 2018– 2019 school year! We are now accepting applications for kindergarten, first, second, third, fourth & fifth grades. To begin the enrollment process, please review the checklist below and complete the attached enrollment application link that follows.

Enrollment Guidelines for Parents and Guardians of New Students

The documents listed below are to be completed and provided to the school within two (2) weeks of the date on the acceptance letter included in this packet.

Admissions Form

Official Release of Records Form
  • This is a legal document requesting your child’s records from their previous school. We request these records no later than within seven (7) days of the first day of school. You must sign and return this form even if your child has no records.
Health Records
  • You must supply a copy of your son’s immunization card and an up to date physical. Without these records, you child may be removed from school. In addition, though not required, we request any Dental Health Certificate available for the child. A list of dentists who provide services on a free or reduced basis to children is available from the local health department.
Birth Certificate (completed Affidavit of Proof of Age & Identity)
  • You must provide an original, certified copy of your son’s birth certificate to the school. The school will make a photocopy for their files, and return the original to you. A certified copy can be obtained at the County Clerk’s Office in the county in which your child was born (a photocopy, hospital or footprint copy is not acceptable).
Proof of Custody
  • If parent names on the birth certificate do not match the person signing the Admissions Form, a court order allocating parental rights, or a marriage certificate if a step-parent, is required
Proof of Residency
  • Current mortgage; lease agreement; gas, water or electric bill (no phone bills accepted or a notarized letter from the person that you reside with. Must contain custodial parent/guardian name.
Social Security Card
  • A photocopy of your child’s government issued Social Security Card.
Photo ID
  • Photocopy of state issued identification card for the person registering the child listed on the application(s).
If we do not receive the above documents within two (2) weeks from the date of the Acceptance Letter, your child may be removed from the school’s accepted list. Before the first day of school, additional forms will need to be completed.
